Ironically, a dimple is a result of muscle deformity on the inside which appears as a dimple on the cheek. So it is purely out of chance that a person may or may not have one.
People who don’t have them even go as far as getting surgeries done on their cheeks to get dimples. But there are lots of other ways to get them which are significantly safer and cheaper.
One can go for piercings or make them appear with contour makeup. There are also some facial exercises which, if done regularly, can create dimples. However, they may not work for everybody. But they are definitely worth a try.
Suck your cheeks in
Simply sucking your cheeks in will create the indent. But it will stay only until you hold them like that. To turn the indent into a dimple, you have hold on like that for as long as you can. It will slowly create the natural dimple.
Press and hold
Pout/Pucker real good and place your finger in the indent so created. And hold like that, longer the better. You can use two of your fingers, or a finger and thumb. Better yet, you can alternate between the combinations.
Place your fingers on the cheeks with a straight face. Now smile while keeping the fingers intact. Re-position your fingers accordingly. This helps you find natural indents and potential dimples. Because usually, a person’s dimples appear when they are smiling wide.
Use a pencil
Our fingers are very soft when compared to a pencil or pen. Also, the surface area of the bottom of the pen is comparatively steeper, so it can apply much more pressure to create the dimple faster and more effectively. It is easier to hold on the cheek as well.

All these exercises are to be done for at least 30 minutes a day. You can choose the one best for y or a combination of all these.
And they will take time, so you have to be really patient. Because you are essentially damaging the tissue to create the natural dimple. So hold on and continue with the exercises.
But the best way to create dimples is to smile and to smile wide. As your natural indents, if any, shrink and form dimples. And you are the most beautiful when you are smiling, with or without a dimple.
